We understand that you, or your child, may feel anxious about visiting the dentist; you are not alone, and we put forth an extra effort to make sure that you are comfortable and happy every time you visit our practice. We are sensitive to your needs as a patient. You can always count on receiving individual attention from our doctors and staff.
Your first visit to our office typically includes a digital x-ray that allows us to view the structure of the jaw, the position of any teeth that have not yet erupted, malformed roots, and tooth decay.
The initial visit also involves getting your medical history. When you share your medical history, be sure to provide complete, up-to-date information on your health. This information will help us select the safest and most effective method of treatment for you.
Click here to download and print the forms you will need for your first visit. Please complete these forms, sign them, and bring them with you to your first appointment. This allows us to use the time allotted for your visit efficiently and effectively.
Please arrive 15 minutes before your first appointment so that we can go over our office policies and your medical history as well as give you a tour of our office.
Bring your smile; but also remember to bring these things with you to your first dental appointment:
- Your completed forms
- Your insurance card and booklet
